LPS completed almost $200 million in Invisible Dental Support Organization (IDSO) partnerships in Q3 for our dental practice clients. Many of these set new record values. With over $5.0 billion of new financing provided to new and recapitalized IDSOs in just the first half of 2024, there are multiple qualified bidders eager to become your silent partner at high values. Note that over $150 million of LPS/IDSO partnerships in the last 24 months were for doctors in their 30s.

And while short-term values are high, more importantly, the largest IDSO in the U.S. just completed its third recapitalization in seven years at a value approaching $4.0 billion. Over 50 very happy LPS doctor clients, who we partnered with this IDSO achieved impressive returns on their retained ownership, with some exceeding 500%.

The political winds are blowing in favor of more IDSO partnerships in 2025. With multiple qualified bidders in an LPS advised process, larger practices should understand their value today.

In July, LPS completed $153+ million of IDSO partnerships for our clients.

Generally in an LPS advised bidding process, practices are valued at between 7.5x EBITDA and 9x EBITDA. However, if you have an exceptional practice with young doctors and growth in 2024, you may achieve a higher value. LPS also gets higher values for our clients when we have multiple clients in a region that choose the same IDSO partner. LPS’ size and insider knowledge add value.

Of the partnerships in July, over $50 million were completed at 11x EBITDA and over $60 million were completed at over 9x EBITDA. Values are still exceptional when you know where to find the right IDSO partner at certain moments in time.

As the dust settles on 2023, I have to say I am surprised that LPS set new record value levels from coast to coast for GPs and specialists of all types. (14.6x EBITDA in one case!). Newly capitalized IDSOs eagerly bid against each other for great practices, unaffected by interest rates. Several 2022 IDSO bidders exited the new partner hunt in 2023 and may or may not be back in 2024.
IDSOs are becoming increasingly selective in their practice partners and practice size and doctor age matter more than ever. Practice growth in 2023 vs. 2022 was critical. Several successful recapitalizations occurred for quality IDSOs and I expect more in Q1 2024. Doctors are continuing to see above market returns on the equity they retain in an IDSO partnership when they choose the right partner.
